About the RoleWe are seeking an experienced Occupational Therapist (OT) Home Health for a 13-week contract serving Alameda County, California. The Occupational Therapist will provide skilled home-based therapy services while prioritizing Start of Care visits, supervising Occupational Therapy Assistants, and maintaining timely clinical documentation and communication.
The position follows a productivity-based home health model with an expected 30 points per week . The therapist will manage a community-based caseload and may be required to expand the assigned territory based on patient care needs.
ResponsibilitiesPerform comprehensive home health occupational therapy evaluations and prioritize Start of Care visits according to clinical and scheduling requirements.
Develop and implement individualized treatment plans designed to improve patient safety, independence, functional abilities, and activities of daily living.
Supervise Occupational Therapy Assistants and coordinate therapy services to support continuity and quality of patient care.
Complete mandatory next-day Start of Care reporting and submit clinical notes and other required documentation within 48 hours of each visit.
Maintain timely communication with supervisors, intake teams, and office staff while meeting the expected productivity standard of 30 points per week.
Minimum 1 Year of Home Health Occupational Therapy Experience Required.
Current, Active California Occupational Therapist License Required.
Current BLS/BCLS Certification Required.
Ability to Supervise Occupational Therapy Assistants and Independently Manage Home Health Start of Care, Evaluation, Recertification, Resumption of Care, Revisit, and Discharge Visits Required.
Valid Driver's License and Reliable Transportation Required for Patient-to-Patient Travel; Ability to Work Monday Friday and Expand the Assigned Territory as Needed Required.
