About the RoleWe are seeking an Occupational Therapist (OT) Home Health for a 26-week contract serving Fairfield, Napa, and surrounding communities in California. New graduates are welcome to apply, and previous home health experience is preferred but not required.
The Occupational Therapist will provide skilled therapy services in patients' homes across the assigned territory. Regular travel is required, and occasional travel outside the primary service area or weekend coverage may be necessary to maintain patient visit expectations.
ResponsibilitiesEvaluate home health patients and develop individualized occupational therapy treatment plans based on functional needs and goals.
Provide skilled occupational therapy interventions designed to improve independence, safety, mobility, and performance of daily activities.
Complete evaluations, reassessments, routine visits, discharge visits, and required clinical documentation according to home health standards.
Educate patients and caregivers regarding therapeutic activities, adaptive techniques, safety strategies, and home programs.
Travel throughout the assigned service territory and outside the primary area when necessary while meeting established visit and productivity expectations.
New Graduate Occupational Therapists May Be Considered; Home Health Experience Preferred but Not Required.
Current, Active California Occupational Therapist License Required.
Current BLS Certification Required.
Valid Driver's License, Current Auto Insurance, and Vehicle Registration Required for Home Health Travel.
Ability to Travel Throughout the Assigned Territory, Work Weekends When Required, Meet Weekly Productivity Expectations, and Complete Required Health and Credentialing Documentation.
